A Truck Mounted Attenuator is safety-critical equipment. If a TMA is involved in an impact, near miss, equipment fault or unusual road-work event, the contractor should create a clear factual record before the vehicle returns to service.

1. Make the Area Safe

Follow the project emergency and traffic-management procedures. Protect workers and road users before focusing on documentation.

2. Record the Time, Location and Work Activity

Note the exact location, shift, work activity, lane arrangement and weather or visibility conditions relevant to the event.

3. Photograph the Vehicle and Attenuator

Capture the vehicle, attenuator, warning equipment, visible damage, road position and surrounding work-zone arrangement. Photographs should be clear and time-linked to the report.

4. Record Operator Information

Document the operator name, shift and a concise factual statement. Avoid speculation in the initial record.

5. Check Equipment Status Before Reuse

A TMA that has been impacted or shows damage should not automatically return to service. Follow competent inspection and manufacturer requirements before redeployment.

6. Record Corrective Actions

List repairs, replacement, inspection, briefing changes or traffic-management changes arising from the event.

7. Preserve the Final Report

Keep incident records with the project safety file, relevant TMA records and any authority or client correspondence.
